Parents, you will now notice that we have posted new medical forms! As of January 1st, 2012 you will be required to have a completed medical form for your student on any student ministry event they leave campus for. There are 2 seperate forms….. 1 for in-town events (in the Triad area), and 1 for out-of-town events. The form for out-of-town events MUST BE NOTARIZED! You will only have to complete one of each of these forms for the whole calendar year. These will be kept in a digital form and will accompany students with each event. We hope this will assist us in being responsible for your student, as well as saving you time and trouble in the event that this information is required for the safety of all of our students. Also, for some big events, there will be another form to fill in and sign, required by the sponsoring organization. Now, about transportation…On church events how is my student transported to and from locations?
Depending on the length of trip or the purpose of the trip, transportation varies. Some trips an adult (21 or above) may transport your student in vehicle with a seatbelt available. If interns (possibly college-age students) will be transporting students. prior approval will be required. Students are not allowed to transport other students during an activity.
They may transport other students home from an activity if they brought them to the activity. If transportation is ever a problem, please call the Student Ministries office and talk to an adult, so we can make sure your student(s) stay safe. However, most trips are in a church vehicle or rented vans driven by an adult 25 years or older.
